Please use the forms available on this course to notify us of any changes at all to your apprenticeship. This can include, but is not limited to:

  • Withdrawing
  • Taking a break in learning
  • Returning to learning after a break
  • Changing employers
  • Changing job roles
  • Change of personal details (contact details, for example)

Completing one of these forms will immediately notify our operations team, who will then contact you regarding next steps. It is vital that you keep us updated regarding any changes throughout your apprenticeship as TVA has a duty to ensure all records and documentation is kept up to date for the ESFA. If you are unsure at any point whether you are required to complete a change of circumstance form, we strongly recommend completing one anyway and operations will advise you if further action is required.

Please note that employers may also complete any of these forms on behalf of their learner. We will then advise the learner that the form has been received and discuss the next steps with all parties involved.
